Navigating the healthcare system, staying on meds, getting health insurance, and reaching substance use goals is harder than it should be! That’s why we have a dedicated team of case managers providing individualized support to people living with HIV.
Once you sign up for case management, your case manager will be available on an ongoing basis to monitor your needs and coordinate services.

You can complete your HIPAA-compliant membership application completely online, on your time. (Need help with your application? Call us at 612.824.LIFE (5433) or contact us by email to arrange a time to complete intake at our community center.)
Join
Meet with our member services team for a tour of our building. We will connect you to the programs and services you need, including food and nutrition services.
