Housing Case Manager will be responsible for assisting individuals in the Hennepin County shelter system obtain safe, affordable housing while utilizing the program’s temporary rental assistance assessing. The case manager will support individuals housed in the program with increasing income while assessing for other needed services to include transportation, chemical health, mental health, and physical health. This position maintains a caseload of 20-30 individuals.
The Kitchen Assistant plays a crucial role in supporting the food preparation and service operations at our community center. This position contributes to our mission by ensuring the efficient and hygienic preparation of meals for individuals living with HIV.
The Program Manager of Case Management provides day to day supervision of Medical and Non-Medical Case Management staff, quality improvement and client access initiatives, implementation of the Medical Transportation Assistance Program, as well as medical case management services to individuals living with HIV/AIDS, many of whom seek help with medication adherence, chemical dependency, mental illness, housing instability, food insecurity, and other issues.

The Benefits Counselor at Aliveness Project spends their day providing health insurance and benefits assistance to Minnesotans living with HIV who are eligible for Minnesota Department of Human Services HIV/AIDS Programs. This role also provides ongoing support and training for other benefits counseling programs and social service providers.
Aliveness Project in partnership with Cardinal Health is looking for a highly motivated HIV-Certified pharmacist interested in managing a entity-owned 340B Retail Pharmacy located within a partner health center. Our pharmacists play an important role in delivering medication and pharmaceutical care in communities that are most in need. This includes immunizations, patient consultation, medication therapy management, and management of high-risk patients.
